    SALARY: $16.00 / hour

    SIGN-ON BONUS OF $750.

    JOB DESCRIPTION

    This program provides services to students who have been expelled from their regular school and who have received approval to attend the Bridges Program. This program is intended to be a short-term interim placement for struggling students who need more one-on-one attention in order to plan for graduation and career preparation.

    KEY RESPONSIBILITIES:

    • Interview and evaluate clients and families to complete intake process.
    • Maintain one-on-one contact in school and family contact as needed.
    • Establish and maintain professional relationships with clients.
    • Monitor client’s attendance, behavior, and grades.  Report these findings to parents and other providers.
    • Analyze client situations, capabilities, and problems to determine services required to meet clients’ treatment needs.
    • Formulate, implement, evaluate, and review individual client’s Plan of Care.
    • Organize and facilitate structured group, individual and/or family activities.
    • Organize and facilitate Community Service Work hours and Service Learning Projects on a weekly basis.
    • Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
    • Encompass The Circle of Courage, Life Space Crisis Intervention, and be an essential part to TIME and Bridges Professional Learning Community.
    • Report monitored client behavior to courts and Kenosha County Department of Human Services, Juvenile Court Intake, Adult Probation, Kenosha Unified School District, and all other community providers and make appropriate recommendations.
    • Compile and monitor all client files, per program standards, to ensure all required documentation is maintained (i.e. assessments, intake paperwork, releases, dispositional reports, IEP’s, BIP’s, HIPAA logs, contact notes and behavior reports).
    • Complete monthly logs, monthly reports, receipts and mileage per program standards.
    • Provide supervision and support to youth placed in the Summer Youth Employment Program.
    • Ability to operate a motor vehicle safely and obey all traffic laws.  Must have reliable transportation on a daily basis, a valid driver’s license and automobile insurance.
    • Safely transport clients as needed.
    • Other duties as assigned.

    QUALIFICATIONS & REQUIREMENTS:

    • Bachelor’s Degree in Social Work, Psychology, Criminal Justice, or other related fields.
    • A valid driver's license, proof of valid automobile insurance, and reliable transportation are required.
    • A background check and drug test are required prior to the offer of employment.
