We are seeking a highly organized, dependable, and compassionate Program Coordinator to support the day-to-day functioning of our residential and community programs. This role is essential to ensuring individuals receive quality services and staff receive the guidance and oversight needed to serve effectively.
Position Summary
The Program Coordinator assists with the oversight of assigned residential homes and community programs, supports staff, ensures person-centered service delivery, and maintains compliance with agency, DDA, COMAR, and CMS regulations. This role works closely with the Program Manager and plays a key part in ensuring programs run smoothly on a day-to-day basis.
This is a hands-on, field-based role that requires strong communication, time management, and organizational skills.
Occasional evenings and weekends required. On-call responsibilities may apply depending on program needs.
Key Responsibilities Individual Support & Planning
Support individuals in their homes and communities to ensure safety, independence, and access to meaningful activities.
Assist with writing, reviewing, and implementing goals, outcomes, and strategies in Person-Centered Plans (PCPs).
Attend annual PCP meetings and support families, staff, and teams in service coordination.
Monitor progress and document outcomes as required.
Program Oversight & Compliance
Assist the Program Manager in overseeing assigned homes/programs.
Ensure services meet COMAR, DDA, and agency standards.
Complete regular home visits, community visits, and compliance checks.
Monitor medical appointments, follow-ups, and ensure documentation is accurate and timely.
Review household funds, logs, daily notes, and incident reports for completeness.
Staff Support & Leadership
Provide coaching, support, and accountability to Direct Support Professionals (DSPs).
Assist with staff scheduling and coverage when needed.
Help train new employees and reinforce expectations, policies, and best practices.
Communicate performance concerns to the Program Manager and assist with corrective plans as needed.
Administrative & Documentation Duties
Complete required documentation accurately and on time, including monthly summaries, progress notes, and follow-up reports.
Ensure timesheets, staff notes, medical documentation, and service logs are correct.
Utilize agency systems (database, scheduling systems, electronic records) for accurate reporting.
Communication & Collaboration
Maintain strong, professional communication with House Manager, DSPs, families, nurses, Program Manager, and other team members.
Attend internal staff meetings, DDA trainings, and agency-required events.
Collaborate with the Program Manager to ensure continuity and quality across programs.
Minimum Requirements
Associate’s degree in Human Services or related field (Bachelor’s preferred).
At least two years of experience supporting individuals with developmental disabilities.
Reliable transportation and active auto insurance.
Valid driver’s license (full license; no provisional).
Clean driving record (no more than 3 points).
Strong documentation and communication skills.
Ability to lift up to 50 lbs if needed.
Preferred Qualifications
Experience in a DDA residential or community program.
Knowledge of COMAR and DDA regulations.
Strong organizational and time management skills.
Experience coaching or leading staff teams.
