    Position Overview

    The Program Coordinator is responsible for the daily oversight, coordination, training, and operational management of the Behavioral and Developmental Services (BDS) and Indiana Pathways programs at Home Care of Indiana, LLC. This role ensures services are delivered with excellence, compliance, and person-centered care. The Program Supervisor works closely with families, caregivers, behavior consultants, case managers, and state agencies to maintain program integrity, support client growth, and support staff in meeting service delivery expectations.

    Standard Work Schedule

    Days: Monday – Friday Office Hours: 7:00 AM – 4:00 PM On-Call Rotation: Every Other Weekend (Bi-Weekly) On-call responsibilities may include responding to urgent client needs, staffing issues, or service interruptions outside standard hours.

    Key Responsibilities :

    Program Operations & Compliance

    Ensure all BDS and Indiana Pathways services follow Medicaid Waiver regulations and agency policies.

    Ensure ongoing CARF compliance and support CARF accreditation standards in daily operations and documentation.

    Maintain accurate, complete, and timely documentation in electronic systems (e.g., Therapy).

    Track service authorizations and utilization to ensure continuity of support.

    Support compliance audits, quality assurance reviews, and corrective action implementation.

    Staff Coordination, Training & Support

    Assist with recruitment, scheduling, onboarding, and oversight of DSPs, and caregiver staff.

    Develop and deliver staff training, including:

    New hire orientation

    Behavior support plan implementation

    Client-specific support expectations

    Pathways and BDS service delivery standards

    Provide ongoing coaching, performance feedback, and corrective training when necessary.

    Promote a supportive and professional work environment to enhance staff retention.

    Client Support & Case Management

    Serve as a primary contact for guardians, families, case managers, and interdisciplinary teams.

    Participate in Person-Centered Planning and treatment team meetings.

    Assist clients in achieving goals related to independence, skill development, and community integration.

    Implement individualized support strategies aligned with clinical recommendations.

    Collaboration & Communication

    Coordinate with Behavior Clinicians, Case Managers, Therapists, and external partners.

    Communicate staffing changes, program updates, and support needs with leadership.

    Respond promptly and professionally to emails, calls, and incident documentation.

    Incident Management

    Review, document, and report incidents in accordance with state and agency guidelines.

    Participate in investigation follow-ups and assist with corrective action planning.

    Qualifications:

    Required:

    High School Diploma or equivalent.

    At least 3 years of experience in behavioral health, human services, or related support roles.

    MUST HAVE A BACK GROUND HISTORY IN BDS OR THE PATHWAYS PROGRAM

    Strong communication, organization, and problem-solving skills.

    Valid driver’s license and ability to pass background checks.

    Preferred:

    Associate or Bachelor’s degree in Human Services, Psychology, Education, or related field.

    Experience supporting individuals with developmental disabilities or behavioral needs.

    Experience providing staff training or team leadership.

    Familiarity with Indiana Waiver Services (BDS, Pathways) and Therapy documentation.

    Annual Salary: $50,000 + Yearly Bonus (Based on client enrollment)
