    Lead one of downstate NY's largest and most impactful care management programs. We're a respected, mission-driven agency serving over 4,000 clients, and we need an experienced leader to drive our expansive Health Homes program.

    This is a rare opportunity to oversee a massive operation and a large multidisciplinary team, ensuring high-quality, coordinated, and compliant care for vulnerable adults and children with complex needs.

     

    Key Responsibilities

    • Direct daily operations for Health Homes Care Management, overseeing service delivery for more than 4,000 clients across adult and children’s programs.

    • Supervise, develop, and support a sizable team of care management supervisors, care managers, and administrative staff.

    • Monitor program metrics, caseloads, and documentation to ensure compliance and drive quality outcomes.

    • Refine and optimize workflows, processes, and use of technology to scale services efficiently while maintaining high standards of care.

    • Serve as the primary liaison with health plans, referral sources, oversight agencies, and community partners.

    • Quickly address challenges—from regulatory shifts to staff or client crises—to ensure program stability and growth.

    • Use data and reporting to guide program improvements and strategic decision-making.

    • Foster a supportive, mission-driven culture that values collaboration, accountability, and ongoing staff development.

     

    Qualifications

    • Significant leadership experience within Health Homes, care management, or Medicaid-managed programs, with expertise in both adult and children’s services strongly preferred.

    • In-depth knowledge of New York State Health Homes regulations, documentation requirements, and best practices in care coordination.

    • Proven ability to manage large teams and complex programs, with a track record of achieving compliance, quality, and measurable outcomes.

    • Strong analytical skills and comfort with data-driven program management and improvement.

    • Dedication to serving high-need populations and the ability to lead with empathy, integrity, and resilience.
