LVNS - LPNS NEEDED FOR ASSISTED LIVING COMMUNITIES IN LOS ANGELES AREA!

POSITION SUMMARY

PROVIDES MEDICATION ADMINISTRATION FOR RESIDENTS AS SPECIFIED IN THEIR INDIVIDUAL SERVICE PLAN. Communicates with physicians and families regarding resident change of condition or variance. Coordinates and schedules work and supervise the delivery of Caregiver service.

ESSENTIAL FUNCTIONS

• Establishes relationships with family and residents and promotes resident satisfaction by:
  • following the resident's individual service plan to ensure health and safety
  • encouraging socialization/ participation in activities among residents
  • assisting with the activity program and chaperone activities outside the residence
• PROVIDES MEDICATION ADMINISTRATION for residents as specified in individual service plan.
• COMMUNICATES WITH PHYSICIANS AND FAMILIES AND PROVIDE UPDATES regarding resident change of condition or variance.
• COMMUNICATES TO DIRECTOR OF RESIDENT CARE or Executive Director changes in residents' condition and/or family concerns
• OVERSEES CAREGIVERS DAILY WORK SCHEDULES to include the development of daily staffing exceptions.

SUPERVISORY RESPONSIBILITIES

• POSITION SERVES AS CAREGIVERS LEAD and assists in maintaining infection control programs and hazard communication throughout the Community

MINIMUM QUALIFICATIONS

• VALID LVN/LPN LICENSE
• Proficient in verbal and written English
• Experience working with residents with cognitive impairment/dementia
• Ability to work with interruptions and among multiple residents
• Ability to work schedules as assigned, including weekends and holidays
• Must successfully complete:
  • Health screening: TB skin test or chest x-ray
  • State fingerprint & FBI background check back to 18th birthday
  • CPR Certification

PREFERENCES

Direct supervisory experience and one year plus of medication administration

Observation and resident care assessment experience

• Personal Computer skills
• Telephone communication skills

PHYSICAL REQUIREMENTS

• Ability to lift, pull or push up to 30 lbs, using proper body mechanics and/or available equipment
• Ability to bend or kneel for up to 15 consecutive minutes
• Ability to work 3- 3.75 hours standing, walking and moving about without need to sit down
