    Job Title: Psychiatric Nurse Practitioner (PMHNP) – Skilled Nursing Facilities

    Location: Norfolk, VA Position Type: Full-Time / Part-Time

    Position Overview A growing healthcare organization is seeking a Psychiatric Nurse Practitioner (PMHNP) to provide behavioral health services to residents in Skilled Nursing Facilities (SNFs). The Psychiatric Nurse Practitioner will work closely with facility leadership, physicians, nursing staff, and interdisciplinary teams to support the mental health and behavioral needs of residents.

    This role focuses on evaluating, diagnosing, and managing psychiatric conditions commonly seen in long-term care populations, while helping improve quality of life and clinical outcomes for residents.

    Key Responsibilities

    Conduct comprehensive psychiatric evaluations and assessments for residents.

    Diagnose and treat mental health conditions including depression, anxiety, dementia-related behaviors, psychosis, and mood disorders.

    Develop and manage psychiatric treatment plans, including medication management and monitoring.

    Collaborate with physicians, nursing staff, social services, and therapy teams to coordinate care.

    Provide ongoing follow-up visits and behavioral health management for residents.

    Assist facilities with behavior management strategies and staff education when appropriate.

    Ensure proper documentation in accordance with regulatory and billing requirements.

    Participate in interdisciplinary care planning and clinical team discussions.

    Support facilities in reducing unnecessary psychotropic medication use when appropriate.

    Qualifications

    Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP).

    Active Nurse Practitioner license in the state of practice.

    DEA license required (or ability to obtain).

    Prior experience in geriatric psychiatry, long-term care, or behavioral health preferred.

    Strong understanding of mental health conditions in elderly populations.

    Excellent collaboration and communication skills.
