About the Role
We are seeking a full-time ACT Psych Rehab Specialist to join our Assertive Community Treatment (ACT) team. This role involves providing community-based, in-home individual and family interventions for clients with mental health and substance use challenges. The focus is on promoting symptom stability, enhancing coping skills, fostering interpersonal relationships, and supporting individuals in achieving their highest level of functioning within the community.

Responsibilities
Client-Centered Interventions:
- Deliver therapeutic interventions based on the person-centered plan.
- Provide individual and family counseling, psycho-educational activities, and behavioral interventions.
- Address daily living skills, work-related services, and leisure-time activities.
Collaboration and Coordination:
- Work with clients, families, natural supports, and collateral agencies to achieve treatment goals.
- Act as a liaison between clients and community resources.
Support and Education:
- Provide training and education to parents, guardians, or natural supports to improve interactions with clients.
- Offer substance abuse treatment and support services to help individuals achieve stability and independence.
Documentation and Compliance:
- Maintain accurate and timely documentation of services, including treatment notes, goals, and plans.
- Ensure compliance with the ACT model as measured by TMACT standards.
Qualifications:
- At a minimum, the ACT Psychosocial Rehabilitation Specialist - QP must have QP status according to 10A NCAC 27G.0104, including a Bachelor's Degree in a human services field with more than two years full-time, post-Bachelor MH/DD/SA experience with the population served; or be a Substance Use Disorder Professional who has two years or more full-time post-bachelor experience in alcoholism and drug abuse counseling or a Bachelor’s Degree in a non-human services field with more than four years full-time, post-bachelor MH/DD/SA experience with population served; or be a Substance Use Disorder Professional who has four or more years full-time, post-bachelor experience in alcoholism and drug abuse counseling.
- Previous experience working with mental health and/or substance abuse clients is preferred.
- Strong organizational, interpersonal, and communication skills.
- Ability to work collaboratively within a multidisciplinary team.
- Must be free from all illicit substance use.
