    ORGANIZATION OVERVIEW

    Across The Life Span, PLLC (ATLS) is an integrated behavioral health, primary care, and opioid addiction treatment organization serving Caswell County and surrounding North Carolina communities under NC Medicaid. We are committed to delivering whole-person, recovery-oriented care across all stages of life through a trauma-informed, culturally responsive clinical model.

    POSITION SUMMARY

    The Qualified Professional (QP) provides direct, community-based behavioral health services to individuals with mental health and/or substance use disorders in accordance with NC Medicaid Community Support Team (CST) and Individual Community Support (ICS) service definitions. The QP works within a person-centered care framework, delivering individualized support, skills training, and care coordination to promote community integration, stability, and recovery.

    This multi-site position serves consumers across the Yanceyville, Roxboro, and Greensboro service areas. The QP functions and collaborates closely with the multidisciplinary ATLS team to ensure continuity of care, documentation compliance, and achievement of individualized treatment goals.

    KEY RESPONSIBILITIES

    Direct Consumer Services

    Provide individualized community support services including skills training in daily living, social functioning, illness self-management, and vocational readiness

    Deliver psychoeducation to consumers and natural supports regarding mental health symptoms, medications, coping strategies, and recovery principles

    Conduct functional assessments and contribute to the development and monitoring of Person-Centered Plans (PCPs) and Individualized Care Plans

    Provide crisis support planning, including development and review of individual Safety Plans in coordination with clinical supervisors

    Assist consumers with identifying and accessing community resources including housing, transportation, benefits, and social supports

    Support consumers in developing self-advocacy skills and engagement with their own recovery process

    Accompany consumers to community appointments, medical visits, and other activities of daily living as clinically indicated

    Care Coordination & Team Collaboration

    Participate in multidisciplinary treatment team meetings, case conferences, and clinical supervision sessions

    Coordinate with primary care providers, psychiatric staff, the ATLS Medical Director, and community partners to ensure integrated, holistic service delivery

    Collaborate with the CPSS peer support team to reinforce recovery milestones and community engagement

    Communicate consumer status updates, safety concerns, and treatment plan changes to the Clinical Services Manager in a timely manner

    Facilitate warm handoffs and referrals to higher levels of care as clinically appropriate

    Documentation & Compliance

    Complete all required documentation in the electronic health record (EHR) within ATLS-established timeframes, including progress notes, treatment plan updates, and service logs

    Ensure Comprehensive Clinical Assessments (CCAs), Person-Centered Plans (PCPs), and service authorizations are completed accurately and submitted within NC Medicaid deadlines

    Maintain documentation in compliance with NC Medicaid Community Support service definitions, HIPAA, and all applicable ATLS policies and procedures

    Participate in internal quality audits and support the QA/QI Officer in achieving compliance benchmarks

    Adhere to ATLS two-level document review workflow protocols

    Accurately track units of service and complete billing documentation in accordance with NC Medicaid requirements

    Professional Development & Organizational Duties

    Complete all required annual and ongoing training including NC Medicaid required competencies, trauma-informed care, and cultural humility

    Maintain current knowledge of NC Medicaid behavioral health service definitions and policy updates

    Participate in new employee orientation activities and staff training as requested

    Fulfill mandatory reporting obligations per NC General Statutes

    Perform other duties as assigned by the Clinical Director or CEO

    MINIMUM QUALIFICATIONS

    To qualify as a Qualified Professional under NC Medicaid definitions, the candidate must meet ONE of the following criteria:

    Bachelor's degree in a human services field (psychology, social work, counseling, sociology, or related discipline) AND two (2) or more years of full-time, post-degree experience with the population served

    Bachelor's degree in any field AND four (4) or more years of full-time experience with the population served

    Associate's degree in a human services field AND four (4) or more years of full-time experience with the population served

    High school diploma or GED AND six (6) or more years of full-time experience with the population served

    Valid North Carolina driver's license and reliable personal transportation (mileage reimbursement provided per ATLS policy)

    Ability to pass criminal background check and other pre-employment screenings

    Proficiency in electronic documentation and Microsoft Office applications

    Strong verbal and written communication skills

    PREFERRED QUALIFICATIONS

    Bachelor's or Master's degree in social work, psychology, counseling, or closely related human services field

    Experience working within NC Medicaid-funded behavioral health organizations

    Familiarity with WellSky, NC Tracks, or similar EHR/billing platforms

    Experience with Community Support Team (CST) or Individual Community Support (ICS) service delivery models

    Knowledge of Motivational Interviewing (MI), trauma-informed care, and recovery-oriented practice frameworks

    Bilingual or multilingual communication abilities

    WORK ENVIRONMENT & CONDITIONS

    This is a community-based position requiring significant travel within Caswell, Person, and Guilford Counties and surrounding areas. The QP will work in varied environments including consumer homes, community settings, and clinical offices across the Yanceyville, Roxboro, and Greensboro locations And surrounding areas. A valid driver's license and dependable transportation are required.
