    Summary Position Statement:

    The Service Coordinator partners with the Staff Manager to facilitate individual and family services for an assigned caseload of individuals who receive Medicaid Waivered and/or grant-related services as designated in a Plan of Care. The Service Coordinator is the primary point of contact for the individual, family, other natural supports, community partners and care coordinators and provides person-centered service planning and implementation by working collaboratively with all team members. The Service Coordinator provides leadership to support quality services which promote inclusion, independence, and well-being including linking individuals with the community and natural supports.

    SECTION III

    Essential Functions of the Position

      1. Promote positive and effective communication and collaboration with all team members. Primary point of contact for the individual and family.
      1. Advocate and coordinate supports as needed for individuals with developmental disabilities. This includes using person-centered tools to develop services as well as linking with community and natural supports in response to the individual's goals, needs and preferences.
      1. Provide assistance and advocacy as needed at appointments and/or meetings – school, medical, therapist, public assistance, etc.
      1. Work closely with the Staff Manager to ensure direct support professionals have support and training needed to successfully provide services to the individuals as assigned.
      1. Assist as needed with day-to-day needs such as budgeting, medication management/assistance, transportation, housing, travel, etc.
      1. Arrange access to resources to address specialized needs such as consultants, assistive technology, medical specialists, etc.
      1. Provide short-term intervention/assistance to families in crisis, and help plan with families to provide long-term supports to avoid future crisis. Assistance may involve home visits, formal mediation sessions, connecting with financial resources, and collaborating with other community agencies such as Alaska Housing, Office of Public Advocacy (OPA), Office of Child Services (OCS), Juneau Youth Service (JYS), Juneau Alliance for Mental Health, Inc. (JAMHI), and the Juneau School District.
      1. Ensure that documentation on services is accurately reported by creating or updating tasks in Therap as goals change, reviewing comments in Therap to evaluate quality of services and developing progress reports.
    1. Maintain and promote a positive professional working relationship with all employees
    2. Document support and services provided in Therap.
    3. Write or assist in writing grants as needed.
    4. Provide assistance and back-up to the Staff Manager as needed.
    5. Maintain client files to be up to date with all paperwork and documentation.
    6. Work irregular hours as needed to accommodate the needs of individuals and families.
    7. Know and follow the REACH Service Principles, Core Values and Mission Statement.

    Position Requirements:

    Requirements: One year of experience providing assistance to individuals with disabilities professional experience in managing recipient needs, preferred. The ideal candidate will have experience working with people that experience developmental disabilities. Bachelor's degree in human services field, or an equivalent combination of education, experience, and training that demonstrates the ability to perform the essential job functions is preferred. CPR and First Aid certificates required within 30 days of hire. TB, fingerprint and background check clearance required. Driving is a requirement of this position, and applicants must be at least 21 years of age, possess a current Alaska Driver's License; have been a licensed driver for not less than 3 years, and provide Agency with proof of current Auto Insurance Coverage and a successful State Motor Vehicle “Full History” Driver's Report, prior to employment.

    Knowledge, Skills & Abilities:

    • Knowledge of the Medicaid system, disability issues, benefits, programs and practices
    • Ability to manage crisis situations using sound judgment
    • Proficiency in the use of computers for word processing, spreadsheets and other applications
    • Ability to organize, evaluate and present information effectively, both orally and in writing, within a prescribed time frame
    • Ability to work with professional and support staff
    • Knowledge of the medical, behavioral, habilitative, and rehabilitative considerations of individuals, and ability to implement services utilizing such knowledge
    • Knowledge of laws, rules, regulations, policies, procedures, precedents and terminology used in the work, client-centered services, knowledge of resources available, applicable state regulations, the Individuals with Disabilities Education Act (IDEA) and Americans with Disabilities Act (ADA)
    • Business or professional writing skills
    • Strong organization, documentation, and time management skills
    • Strong interpersonal and communication skills
    • Demonstrated experience pertaining to the appropriate handling of confidential and privileged information
    • Detail oriented with the ability to handle multiple tasks simultaneously
    • Effective problem-solving skills

    Physical Requirements: Ability to perform sedentary work. Visual acuity necessary to utilize a computer monitor throughout the course of a normal workday. Ability to perform repetitive keyboarding activities and operate general office equipment. Ability to communicate verbally, both in person and over the telephone.  May come in contact with blood borne pathogens.  Heavy lifting may be required.

    Working Conditions:  Work is performed at the REACH office and in the field.
