SUMMARY POSITION STATEMENT:
This position is responsible for providing early intervention and occupational therapy services to families with infants and toddlers (birth to 3 years of age) who experience developmental delays or disabilities. Services include transdisciplinary play based developmental evaluations, family service coordination, and occupational therapy. Evaluations, Individualized Family Service Plans (IFSP), and home visits are completed and implemented with families to promote the family's capacity to support their child's growth and development within the child and family's natural environment.

POSITION REQUIREMENTS:
Licensure: Current State of Alaska Occupational Therapy or license eligible preferred. Will also consider other licensure in mental health such as LPC or LMFT or new graduates. Registry with the SEED with an assigned Level 10 or higher within 30 days of hire. Completion of the State of Alaska Part C Credential within 6 months of hire.

Minimum Education: Bachelor's or Master's degree in occupational therapy required. CPR and first aid certificates required within 30 days of hire. TB, fingerprint and background check clearance required. Must have current Alaska driver's license and auto insurance per agency standards.
Minimum Experience: 1 year of pediatric occupational therapy experience required.
Knowledge, Skills & Abilities:
Working Conditions: Work is performed at the REACH office, client homes, and requires traveling to home visits, evening visits and may include travel to outlying communities by small planes.
Provides family service coordination for children enrolled in the program.
Develops and implements Individual Family Service Plans.
Maintains up-to-date and accurate files and data entry in accordance with ILP and agency policies and follows billing procedures.
Participates as a team member in transdisciplinary assessments.
Travels to outlying areas as needed to provide services.
Develops and maintains collaborative working relationships with other providers and agencies serving families. In doing so, makes referrals and accesses resources for children and families.
Work is performed primarily within the client's home, including evening home visits; and will require driving during inclement weather conditions.
Facilitates the transition process for children exiting from the program based on ILP standards.
Actively participates in on-going learning, mentoring, and relationship-based supervision.
