    NURSE PRACTITIONER JOB RESPONSIBILITIES:

    The Nurse Practitioner serves as an integral member of the treatment team collaborating to improve health outcomes and promote an integrated care model.

     

    • Provides Primary Care, routine, and acute care, for scheduled, same day and walk in appointments.
    • Demonstrates proficiency of electronic medical record, including documentation, e-prescribing, and coding.
    • Complies with productivity expectations under the direction of the Chief Medical Officer.
    • Initiation of treatment and management of chronic illnesses including, but not limited to, HIV infection, asthma, bronchitis, diabetes, lipid disorders, and hypertension.
    • Generates appropriate referrals to ancillary and specialty services including medical, mental health and social based services.
    • Maintains documentation and e-prescribing practices according to standards of the agency and regulatory agencies including but not limited to history and physical, social history and literacy level as well as care management.
    • Completes and locks progress notes within 72 hours of client encounter.
    • Ensure timely prescribing, orders, addressing labs, interpreting of test results, addressing telephone encounters, ordering, and reviewing of specialty referrals and consultation reports.
    • Performs routine diagnostic and therapeutic procedures according to accepted practice standards.
    • Discusses and refers complicated patients outside of clinical expertise to appropriate physician.
    • Provide health education and counseling during visits.
    • Provides on-call telephone coverage on a rotating schedule.
    • Participates in CQI activities.
    • Other duties as assigned by the Chief Medical Officer/Chief Clinical Officer.
    • Participate in Job Training and Education Programs, Meetings and Committees.

     

    35-hour work week and offers competitive benefits. The Nurse Practitioner will have flexibility to set the hours worked; however, work schedule must meet the needs of the health center.

    • Experience using Microsoft Office suite and Outlook
    • Bilingual Spanish is a plus

    Education, Experience, and Licensing Requirements:

    • Completion of a family medicine program in an accredited institution
    • Current BC in Family Medicine (American Association of Nurse Practitioners or equivalent)
    • Valid New York State medical license as FNP
    • Valid DEA certificate with DATA waiver
    • Minimum 5 years’ experience working full-time as a family nurse practitioner
