Nutritionist – Weight Management & Chronic Disease Care (Diabetes, Hyperlipidemia)
Practice Overview
We are a well-established, patient-centered family medicine practice seeking a Nutritionist to join our multidisciplinary team. This part-time or full-time role will involve working with both adults and children to manage weight, improve dietary habits, and assist with chronic disease management, particularly diabetes and hyperlipidemia. Spanish speaking is a must.
Position Summary
The Nutritionist will provide nutritional counseling, meal planning, and education to patients managing chronic conditions. You will work closely with physicians and other healthcare providers to develop tailored nutrition plans that support patients in improving their overall health, managing chronic illnesses, and achieving their wellness goals.
Key Responsibilities
Provide one-on-one nutritional counseling to adults and children
Assist patients with weight management strategies, including healthy eating, portion control, and behavior modification
Develop and implement personalized nutrition plans for patients with diabetes, hyperlipidemia, and other chronic conditions
Educate patients on healthy eating, reading food labels, meal planning, and managing dietary needs for chronic illness
Monitor patients' progress and adjust nutrition plans as needed
Collaborate with physicians to track patient outcomes and provide comprehensive care
Lead group workshops or seminars on topics related to nutrition, wellness, and chronic disease management
Maintain accurate documentation and progress notes in the electronic medical record (EMR)
Stay current on nutrition research and evidence-based practices
Qualifications
Degree in Nutrition, Dietetics, or related field (BS, MS, or equivalent)
Registered Dietitian (RD) or eligible for registration (preferred)
State licensure (if applicable)
Strong knowledge of chronic disease management, particularly diabetes, hyperlipidemia, and obesity
Experience in both adult and pediatric nutrition counseling
Ability to develop and communicate effective nutrition plans
Compassionate and patient-focused, with excellent communication skills
Experience with nutrition therapy for chronic illnesses is a plus
