    This is a part time/direct care position. The incumbent in this position is responsible for providing direct services to individuals in the group home or day support according to directions and scheduling provided. Scheduling for this position is based on the needs of the program individuals and/or the program. This position reports to the Program Manager.

    Major Duties:

    As assigned , provides support as indicated in client's Individual Service Plan including training to assist clients in meeting objectives, support and assistance as needed with personal care, domestic skills, community integration and/ or related residential support activities. Implements activity schedule.

    Provides behavioral support in accordance with Person Center Plan (s) and in compliance with company policy and regulations.

    Helps and supervision with transportation for appointments and activities; use of personal vehicle as may be required.

    Participates in interdisciplinary team meetings to discuss the development of the Person-Centered Plan or the progress towards meeting stated outcomes.

    Completes the required documentation of programs activities and ensures that the individual's record is up to date.

    Maintains , as necessary, the required reports to supports payroll and billings.

    Ensures the residences are well maintained; reports any maintenance, health, or safety concerns promptly.

    Monitors health and general well being of individuals, including possible side effects of medications;assists in the administration of medication in accordance with procedure. Other duties as assigned may vary by house manager.

    Knowledge of:

    Principles and philosophy of community mental retardation service.

    Concepts of positive behavioral supports.

    Specialized and general knowledge of the implementation of individual training programs.

    Documentation and data collection systems.

    Licensure regulation include human rights policy and other standards and laws.

    First Aid, CPR, infection control, medication, and behavior management.

    Emergency response procedures.

    Skills at:

    Good oral and written communication.

    Interpersonal skills particularly in terms of building trust and rapport with person who have speacial needs.

    Observing and documenting behavior.

    Responding to the special needs of people with disabilities.

    Implementing and documenting training programs for special needs populations.

    Ability to:

    Use common sense approaches to implement services.

    Carry out and document training and behavior support programs.

    Appropriately implement professional confidential practices.

    Demonstrate commitment to quailty, community base services for people with disabilities

    Communicate to others the nature and philosophy of the services offered.

    Work various shifts and locations to meet the needs of the clients and/or the program.

    Do tasks that require lifting, bending, running, and kneeling. Must be able to lift up to 50 pounds.

    Minimum Training and Experience:

    High school diploma/GED,PCP,CNA or 1year of experience working in the human service field or can demonstrate through a combination of education and/or experience the knowledge and ability to provide training in independent living skills, community integration skills and related skills and activities.

    Must successfully complete the required Medicaid Waiver Training, Human Rights Training, and Medication Administration Training.

    Must maintain current certification in CPR, First Aid, or Management of Aggressive Behavior.

    Must meet basic personnel requirements for Criminal History, Child Protective Services and Department of Motors Vehicles.

    Must obtain an evluation indicating the absence of TB in a communicable form.

    Must have reliable/avaiable transportation and maintain an accetable driving record if ask to transport program individuals.

    Must maintain an active/correct telephone for company contact purposes.
