    Where You'll Shine:

    As an RN with ComForCare, you’ll play a vital role in delivering high-quality, personalized care while supporting positive outcomes for our clients and their families.

    Working within your state’s Nurse Practice Act you will:

    Provide services in accordance with the care plan

    Make the initial evaluation visit and regularly reevaluate the patient’s needs

    Initiate the plan of care and necessary revisions

    Coordinate services

    Inform staff of changes in the condition and needs of the client

    Prepare clinical and progress notes as well as summaries for each patient visit

    Understands and adheres to established ComForCare policies and procedures

    Take urgent calls from caregivers, clients and client families when necessary

    Assist with medication management

    Monitor and document vital signs and health status

    Educate clients and families on care plans

    Accurately document care and observations

    Serve as the bridge between non-medical support and skilled nursing care

    Your compassion and expertise help ensure safety, dignity, and consistency for every client you serve.

    You’ll Thrive Here If You Are/Have…

    A Graduate of an approved school of professional nursing

    An active, in-good-standing RN license in the state of practice

    Two (2) to Five (5) years of nursing experience

    Two (2) to Five (5) years of case management experience

    Two (2) to Five (5) years of RN administrative experience

    A genuine passion for working with seniors

    A flexible, positive, and energetic attitude

    Prior RN experience in Assisted Living or Memory Care (preferred)

    A professional image in both appearance and behavior

    Strong written and verbal communication skills
