Position Title: Registered Nurse (RN)
Department: Client Assessment
Position Summary
The Registered Nurse (RN) is responsible for the clinical oversight of client care services. The RN conducts initial nursing assessments, develops individualized care plans, supervises direct care staff, and performs scheduled reassessments of clients at required intervals including 30-day and 90-day reassessments to ensure quality care and regulatory compliance. Qualifications
The Registered Nurse must meet the following qualifications:
• Hold a current, active Registered Nurse license in the State of South Carolina
• Graduate from an accredited nursing program
• Minimum one (1) year nursing experience preferred in home health or community health
• Knowledge of home care regulations and standards
• Ability to assess physical, emotional, and social needs of clients
• Strong documentation and communication skills
• Current CPR certification preferred
• Valid driver’s license and reliable transportation
Essential Duties and Responsibilities
Client Assessments
The Registered Nurse shall:
• Conduct initial comprehensive nursing assessments prior to the start of services
• Evaluate the client’s physical condition, mental status, safety needs, and home environment
• Identify medical conditions, medications, and functional limitations
• Determine appropriate level of care and services required
• Develop an Individualized Plan of Care (POC) in coordination with the client and family
• Ensure services are appropriate for the client’s needs
30-Day Reassessment
The Registered Nurse shall perform a 30-day reassessment for new clients to:
• Evaluate the effectiveness of the current care plan
• Assess changes in the client’s physical or mental condition
• Review caregiver performance and service delivery
• Identify any new medical concerns or risks
• Update the care plan if necessary
• Ensure services remain appropriate and safe
90-Day Reassessment The Registered Nurse shall conduct 90-day reassessments to: • Review the client’s overall health status
• Evaluate progress toward care plan goals
• Assess safety in the home environment
• Review medications and medical changes
• Confirm that services remain necessary
• Update or revise the care plan as needed
• Document findings and recommendations
Plan of Care Responsibilities
The RN is responsible for:
• Developing individualized care plans for each client
• Updating the care plan after reassessments
• Communicating care plan changes to caregivers
• Ensuring caregivers follow the approved care plan
Staff Supervision
The Registered Nurse will:
• Provide clinical supervision to caregivers and home care aides
• Conduct supervisory visits in the client’s home
• Evaluate staff performance and adherence to care plans
• Provide education and guidance to caregivers
• Report performance concerns to the Administrator
Documentation Requirements
The RN shall ensure:
• Accurate and timely documentation of all assessments
• Completion of reassessment reports
• Documentation of care plan updates
• Compliance with agency documentation policies
• Records are maintained according to regulatory requirements Quality Assurance
The RN participates in agency quality improvement activities including:
• Monitoring client satisfaction and outcomes
• Reporting incidents or changes in condition
• Ensuring compliance with agency policies and state regulations
• Assisting with audits and licensing reviews
Client Rights and Safety
The Registered Nurse must:
• Respect the dignity and rights of all clients
• Maintain confidentiality of client information
• Ensure safe care practices in the home
• Report abuse, neglect, or exploitation according to state law
Physical Requirements The RN must be able to: • Travel to client homes
• Perform physical assessments
• Lift up to 25 pounds when necessary
• Sit, stand, and walk for extended periods
Work Environment
Work is performed in client homes, agency offices
