Salary and Benefits – RN Case Manager – Eastern Orange County Territory

• $87,000 - $105,000 (Based on Experience)
• PTO Up to 136.5 Hours/Year In The First Four Years
• Increases To 174 Hours/Year After Five Years
• 6 Paid Holidays
• Full Medical, Dental, and Vision Insurance
• Mileage Reimbursement: Paid From Patient To Patient

Key Responsibilities – RN Case Manager

• Monday To Friday: 8:30 AM – 4:30 PM
• Evening and Weekend Call Rotation: Approximately Every 5-6 Weeks
• Provide Skilled Nursing Care and Symptom Management in Patient Homes
• Develop and Implement Individualized Care Plans With the Hospice Team
• Educate Patients and Families on Care, Medications, and Disease Management
• Coordinate Care with Physicians and Interdisciplinary Team Members
• Document Patient Care Accurately with EMR – Healthwise

Requirements:

• Active New York RN License
• 1 Year of Hospice or Palliative Care Experience -Preferred
• 5 Years of Experience as an RN
• Familiarity with EMR System with Healthwise
