About the Role:

Responsibilities:

Conduct comprehensive patient assessments to develop personalized care plans.

Coordinate and manage patient care activities across interdisciplinary teams.

Monitor patient progress and adjust care plans as needed to ensure optimal outcomes.

Provide education and support to patients and their families regarding health management.

Document all patient interactions and care plans in compliance with agency policies.

Collaborate with physicians and other healthcare professionals to enhance patient care.

Participate in quality improvement initiatives to enhance service delivery.

Ensure adherence to regulatory requirements and best practices in home health care.

Requirements:

Current RN license in the state of Indiana.

Minimum of 2 years of nursing experience, preferably in home health or case management.

Strong assessment, organizational, and communication skills.

Ability to work independently and manage a diverse caseload.

Experience with electronic health records (EHR) systems.

Compassionate demeanor with a focus on patient-centered care.

Basic Life Support (BLS) certification required.

Knowledge of Medicare and Medicaid regulations is a plus.
