Job Description
What You’ll Do:
Leadership & Supervision
· Directly supervise and mentor LVNs/LPNs/RNs in the field according to state guidelines
· Provide clinical oversight, training, and ongoing support to homecare staff
· Conduct performance evaluations and competency assessments
Care Coordination & Clinical Oversight
· Coordinate and oversee the implementation of physician-directed Plans of Care
· Complete admissions, recertifications, resumptions of care, and all required clinical documentation
· Provide patient and caregiver education to promote health and wellness
· Monitor for changes in condition and communicate effectively with interdisciplinary teams
Compliance & Quality Assurance
· Ensure care delivery complies with MGA, state, and federal regulations, as well as accreditation standards
· Review documentation for completeness, accuracy, and reimbursement compliance
· Participate in quality improvement initiatives, risk management, and reporting
Collaboration & Program Development
· Partner with Client Services Managers and the Director of Clinical Services to enhance client satisfaction and clinical outcomes
· Participate in strategic initiatives to support operational growth and patient care excellence
Qualifications
What You’ll Bring:
· Active RN license in the state of practice (BSN preferred)
· Minimum 2 years of clinical nursing experience (Home Health or Acute Care preferred)
· Demonstrated leadership and supervisory skills
· Strong communication, organization, and critical thinking abilities
· Familiarity with clinical documentation requirements and compliance standards
· Current CPR certification, valid driver’s license, and reliable transportation
