We’re looking for a per diem Home Health Case Manager who’s organized, clinically skilled, and ready to join a supportive team that values autonomy, teamwork, and work-life balance.

    What You'll Be Doing

    Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.

    Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.

    Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.

    Monitor and Evaluate: Track patient progress, update care plans, and communicate with families and caregivers as needed.

    Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.

    Advocate for the Patient: Provide education, resources, and emotional support to patients and their families to promote independence and wellness.

    Infusion therapy: Provide skilled nursing services in accordance with physician orders, Plan of Care, and agency policies. Perform and manage infusion therapies including, but not limited to: IV antibiotics, Hydration therapy, TPN, Biologics and specialty medications

    Assess, access, and maintain vascular access devices (PICC lines, midlines, ports, peripheral IVs).

    Monitor patient response to infusion therapy and promptly identify and manage complications (e.g., infiltration, extravasation, infection, adverse reactions).

    Implement infection prevention and control practices consistent with CMS and ACHC standards.

    Educate patients and caregivers on infusion therapy, medication administration, line care, and safety precautions, documenting understanding and return demonstration when applicable.

    What We’re Looking For

    License: RN (Registered Nurse) with active MA state license.

    Experience: At least 1 year of experience in home health or case management preferred. Expertise in administering IVs and medications.

    Skills: Strong critical thinking, communication, time management, and the ability to work independently.

    Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).

    Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.
