    Client Care Manager – Home-Based Elder Care | Full-Time | Central Maryland

    Location: Rockville, MD (Local travel required – up to 75%)
    Employment Type: Full-Time
    Compensation: Salary + Monthly Bonuses

We're looking for a compassionate, experienced Client Care Manager to join our team and help us deliver exceptional home care services to aging clients and their families. This role offers the chance to make a real difference, supporting high-quality, concierge-based elder care with a focus on dignity, independence, and personalized support.

    What You'll Do:

    • Serve as the primary relationship manager for clients and their families
    • Conduct in-person client assessments and reassessments to evaluate care needs
    • Recommend personalized care plans to enhance quality of life
    • Guide clients and families through the onboarding process to initiate services
    • Maintain accurate and timely care documentation and updates
    • Communicate proactively with internal care teams, caregivers, and external providers
    • Match clients with appropriate caregivers and oversee introductions
    • Monitor and adjust client services and caregiver training as needs evolve
    • Resolve client or caregiver issues and complaints with professionalism and discretion
    • Conduct home safety assessments and ensure quality assurance standards are met
    • Adjust client rates as needed based on care changes or satisfaction levels
    • Support compliance with state regulations and internal policies
    • Identify cross-referral or service expansion opportunities
    • Participate in on-call and after-hours support as needed
    • Other duties as assigned

    Qualifications:

    Required:

    • Bachelor's Degree in Human or Health Services or related field (or equivalent work experience)
    • Excellent communication, customer service, and conflict resolution skills
    • Proficiency in documentation systems and general computer skills
    • Valid driver’s license and proof of insurance
    • Ability to lift and carry up to 20 pounds
    • Ability to sit, stand, and walk for extended periods

    Preferred:

    • 3+ years of experience in health care, elder care, social work, or a related industry
