RN Clinical Director / Director of Nursing – Home Health

A Passionate Helping Hand Home Healthcare LLC is seeking an experienced, dependable, and motivated Registered Nurse (RN) Clinical Director / Director of Nursing to join our leadership team.

We are looking for a strong clinical leader who is organized, proactive, knowledgeable in home health, and prepared to take ownership of clinical operations. The ideal candidate will work closely with agency leadership while providing clinical oversight, supporting nursing staff, maintaining regulatory compliance, and helping ensure patients receive high-quality, patient-centered care.

This is an excellent opportunity for an experienced home health RN who enjoys leadership and wants to play an important role in strengthening and growing a home healthcare organization.

Key Responsibilities

Provide leadership and oversight of day-to-day clinical operations

Supervise, support, and provide direction to clinical staff

Oversee admissions, Start of Care (SOC), recertifications, resumptions of care, and discharges

Monitor Plans of Care, physician orders, and required clinical documentation

Review clinical records for accuracy, completeness, timeliness, and compliance

Provide oversight of OASIS documentation and clinical processes

Monitor patient care and address clinical concerns promptly

Coordinate with physicians, patients, caregivers, clinical staff, and agency leadership

Provide staff education, coaching, and clinical guidance

Monitor staff performance and promote accountability and timely completion of responsibilities

Participate in QAPI and performance-improvement activities

Assist with clinical audits, regulatory reviews, and survey preparation

Maintain organized, compliant, and survey-ready clinical records

Identify opportunities to improve clinical workflows, patient outcomes, and overall agency operations

Qualifications

Current and unrestricted Indiana Registered Nurse (RN) license

3–5 years of nursing experience preferred

Home health experience strongly preferred

Previous experience as a Director of Nursing, Clinical Director, Clinical Manager, Nursing Supervisor, or similar leadership role preferred

OASIS experience strongly preferred

Knowledge of Medicare and Medicaid home health requirements preferred

Strong clinical documentation and record-review skills

Excellent leadership, communication, organizational, and problem-solving abilities

Ability to prioritize responsibilities and work independently

Experience with electronic medical record systems; Axxess experience is a plus

Ability to provide clinical guidance and maintain appropriate staff accountability in a professional and supportive manner
