RN Home Assessment (Per-Case Basis)

We are seeking a compassionate and detail-oriented Registered Nurse (RN) Clinical Supervisor to support our home care agency on a per-case basis. This flexible, field-based role involves conducting in-home client assessments, developing individualized care plans, supervising and evaluating CHHAs/CNAs, and performing follow-up visits for active clients as needed.
This position offers the opportunity to play a key role in maintaining high standards of care and has the potential to transition into a full-time clinical leadership role.
Compensation: Compensation is provided per completed services, including:
In-Home Client Assessments (Initial Evaluation)
Re-Evaluation/Follow-Up Visits
Caregiver Training and Skills Evaluation
This position operates on a per-visit basis with flexible scheduling; minimum hours are not guaranteed.
Key Responsibilities
Clinical Evaluations: Perform initial and ongoing physical and mental health assessments, including environmental safety checks (e.g., fall risks, sanitation, and home hazards).
Care Planning: Develop, implement, and update individualized care and service plans based on client needs and clinical findings.
Patient & Family Education: Provide education on disease management, medication awareness, self-care techniques, and home safety procedures.
Caregiver Supervision & Training: Provide clinical guidance, skills training, and performance feedback to caregivers to ensure safe and effective care delivery.
Care Compliance Monitoring: Conduct supervisory visits to monitor care quality and ensure care plan adherence.
Incident Reporting & Risk Management: Identify, document, and report incidents, safety concerns, or changes in client condition following agency protocols.
Documentation & Compliance: Maintain accurate documentation of client visits, assessments, trainings, and evaluations in accordance with agency and regulatory requirements.
Regulatory Compliance: Ensure services comply with New Jersey State home care regulations and agency policies.
Clinical Operations Support: Support agency launch through operational development and community outreach.
Qualifications
Active Registered Nurse (RN) license in the State of New Jersey (Required)
Minimum 2 years of nursing experience, preferably in home care, community health, or case management (Required)
Current BLS/CPR certification (Required)
ACLS and/or PALS certification (Preferred)
Bachelor of Science in Nursing (BSN) preferred; MSN is a plus
Experience with clinical assessments, care plan development, and patient education.
Prior experience in a supervisory, case management, or caregiver oversight role preferred.
Strong clinical documentation skills and knowledge of healthcare regulatory compliance
Valid NJ Driver’s License, reliable transportation, and ability to travel locally for in-home visits; must meet required medical and background clearance guidelines
Physical Requirements:
Ability to sit, stand, and work on a computer for extended periods
Ability to occasionally lift or carry up to 15 lbs (e.g., nursing bag or equipment)
