    How You’ll Lead & Serve

    In-Home Clinical Oversight

    Complete start-of-care assessments and craft personalized, compliance-ready Care Plans.

    Revisit clients on-site to update goals and interventions as conditions change.

    Caregiver Coaching & Compliance

    Orient new aides to each Care Plan; perform skills check-offs and annual competencies.

    Audit field documentation, spot gaps, and provide real-time feedback that elevates care quality.

    Recruitment & Retention Partner

    Collaborate with HR to interview, select, and mentor top-tier caregivers; champion a culture of recognition and growth.

    Documentation & Reporting

    Maintain HIPAA-secure records, track condition changes, and escalate concerns to physicians and leadership.

    Family & Community Liaison

    Build trusting relationships with clients, their support networks, and referral sources; represent our agency’s mission wherever you go.

    What You’ll Bring

    Active Registered Nurse (RN) license in NJ.

    Bachelor’s in Nursing or related field – OR – 2 + years’ geriatric / home-care case-management experience.

    Proven ability to juggle multiple home visits, phone triage, and documentation with precise time management.

    CPR/First-Aid certification and valid driver’s license.

    Warm, decisive communicator who inspires confidence in caregivers, clients, and families alike.
