    Job Description

    Are you a compassionate and detail-oriented Registered Nurse looking to make a meaningful impact in the lives of others? Freedom Home Care is seeking a dedicated RN Care Manager to join our team at our Twin Cities Branch to be scheduled 35 hours per week. In this role, you will be responsible for creating, managing, and supervising individualized plans of care for our clients while working collaboratively with caregivers and office staff.

    As an RN Care Manager, you will play a critical role in maintaining high standards of care, supporting caregivers, and ensuring clients receive safe, personalized services in their homes. This position blends clinical knowledge with strong communication, administrative, and organizational skills to promote excellent outcomes for those we serve.

    Key Responsibilities

    Care Management & Clinical Oversight

    Conduct initial assessments and develop written care plans in collaboration with clients and/or their designated family members.

    Review and update care plans as needed, ensuring ongoing accuracy and appropriateness of care.

    Supervise client care provided by Freedom Home Care caregivers, ensuring adherence to care plans.

    Ensure all care documentation, including service agreements and care plan approvals, is completed and maintained in client records.

    Client & Family Communication

    Maintain regular contact with clients and families to monitor satisfaction and address concerns.

    Communicate changes in client condition to appropriate parties and modify care plans accordingly.

    Educate clients and families about services, expectations, and care routines.

    Team Coordination & Compliance

    Ensure caregivers review care plans prior to providing services and are following the plan as written.

    Support caregiver training and provide clinical guidance when needed.

    Collect applicable fees and ensure appropriate documentation is provided at the start of care.

    Administrative Duties

    Coordinate and manage caregiver schedules to ensure timely and appropriate client coverage.

    Assist with internal and external marketing initiatives to promote services and support referral growth.

    Collaborate with office staff to streamline administrative workflows and maintain regulatory compliance.

    Monitor and report on performance metrics related to client satisfaction, service delivery, and care quality.

    Maintain organized records and ensure timely completion of required documentation and reports.

    Qualifications

    Education: Bachelor’s degree in nursing, social work, psychology, gerontology, health and human services, or related field OR at least 2 years of experience in geriatrics and/or care management.

    Licensure: Current Registered Nurse (RN) license in the state of Minnesota.

    Experience: Previous RN or care management experience preferred.

    Skills: Strong interpersonal, organizational, and communication skills. Proficiency in clinical documentation and administrative processes.

    Other Requirements

    Valid driver’s license

    Current First Aid/CPR certification
