    About the Role:

    Join ProRehab HealthCare Group of Companies as an RN in a home health setting, where you'll provide compassionate care to patients in the comfort of their own homes.     Responsibilities:

    Conduct comprehensive patient assessments and develop personalized care plans.

    Administer medications and treatments as prescribed by physicians.

    Monitor patient progress and document changes in health status.

    Educate patients and families on health management and wellness strategies.

    Coordinate with interdisciplinary teams to ensure holistic patient care.

    Provide emotional support and guidance to patients and their families.

    Maintain accurate and timely documentation in compliance with healthcare regulations.

    Participate in ongoing training and professional development opportunities.

    Requirements:

    Current RN license in the state of California.

    Minimum of 1 year of nursing experience, preferably in home health or community settings.

    Strong clinical assessment and critical thinking skills.

    Excellent communication and interpersonal skills.

    Ability to work independently and manage time effectively.

    Compassionate demeanor with a patient-centered approach.

    Basic Life Support (BLS) certification required.

    Familiarity with electronic health records (EHR) systems is a plus.
