Responsibilities:
Provide direct patient care in home settings, including assessments, treatments, and medication administration
Develop, implement, and monitor individualized patient care plans
Perform wound care, IV therapy, catheter care, and other skilled nursing procedures as needed
Monitor and document patient health status, progress, and changes in condition
Communicate effectively with physicians, care coordinators, and family members regarding patient care
Educate patients and caregivers on disease management, medications, and home safety
Maintain accurate and timely clinical documentation in compliance with state and federal regulations
Requirements:
Active RN or LPN license in the state of Ohio in good standing
Previous home health, long-term care, or clinical nursing experience preferred
Strong assessment, critical thinking, and clinical decision-making skills
Excellent communication and interpersonal skills with patients and families
Reliable transportation and valid driver's license for home visits throughout Columbus, OH
Ability to work independently with minimal supervision
CPR/BLS certification required or ability to obtain upon hire
