Prestige Home Health Care LLC is looking for a compassionate and skilled Registered Nurse (RN) to join our dedicated team in Phoenix, AZ. Responsibilities:
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, treatments, and skilled nursing procedures in a home setting
Monitor and document patient progress, vital signs, and changes in condition
Coordinate care with physicians, therapists, and other healthcare team members
Educate patients and family members on disease management, medications, and home care techniques
Ensure compliance with state, federal, and agency regulations and standards of care
Maintain accurate and timely clinical documentation using electronic health records (EHR)
Requirements:
Active Arizona RN license in good standing
Current BLS/CPR certification required
Minimum 1 year of clinical nursing experience; home health experience preferred
Strong knowledge of skilled nursing procedures, wound care, and IV therapy
Excellent communication, time management, and organizational skills
Ability to work independently and manage a patient caseload efficiently
Valid driver's license and reliable transportation for home visits throughout Phoenix, AZ
