    Quality Improvement & Certification Coordinator – Heart Failure & Stroke Programs

    Join a mission-driven team dedicated to advancing excellence in patient care. We’re seeking a passionate and detail-oriented Quality Improvement & Certification Coordinator to lead efforts in maintaining and achieving certifications of excellence in Heart Failure (HF) and Stroke care. This high-impact role blends strategic quality initiatives, cross-functional collaboration, and clinical data expertise to drive continuous improvement and ensure alignment with top-tier accreditation standards.

    What You’ll Do

    • Lead Certification & Compliance Initiatives
      Coordinate and oversee all activities, metrics, and standards related to Disease Specific Certifications (HF and Stroke), ensuring compliance with accrediting body requirements and advancing best practices in care.

    • Drive Quality Improvement (QI) Strategy
      Lead or collaborate with department-specific or system-wide QI teams to design and implement data-driven performance improvement efforts that directly impact patient outcomes.

    • Data Stewardship & Insights
      Collect, analyze, and present quality and performance data including core measures, hospital/medical staff quality metrics, complaint trends, and peer review data specific to HF and Stroke. Use findings to support strategic planning and drive improvement.

    • Educate & Empower
      Serve as a subject matter expert and educational resource for clinical and non-clinical staff. Provide ongoing training and orientation on disease-specific standards, quality measures, and compliance best practices.

    • Accreditation & Survey Leadership
      Lead accreditation readiness and survey coordination activities. Champion hospital-wide adherence to certification standards and proactively address potential compliance gaps.

    • Champion Interdisciplinary Collaboration
      Actively participate in interdisciplinary rounds to identify care improvement opportunities and support integrated approaches to patient management.

    • Community Engagement & Outreach
      Represent the organization in HF and Stroke-related community outreach efforts, helping extend our impact beyond hospital walls and strengthen our reputation as a care leader.

    Who You Are

    • A motivated healthcare professional with experience in quality improvement, clinical data management, or certification/accreditation processes.

    • Adept at translating data into actionable insights and leading collaborative initiatives.

    • A confident communicator and educator with a passion for improving healthcare systems.

    • Comfortable working cross-functionally with clinical, administrative, and external stakeholders.

    Preferred Qualifications

    • RN, BSN, or other relevant clinical credentials (strongly preferred)

    • Experience with Joint Commission Disease-Specific Certification, Get With The Guidelines, or similar programs

    • Knowledge of QI methodologies (e.g., PDSA, Lean Six Sigma)

    • Experience with EHR systems and data collection platforms
