    RN Senior Director of Case Management
    Orlando, Florida

    The RN Senior Director of Case Management serves as a subject matter expert in Case Management, demonstrating solid leadership, trust-building, team building, and Change Management abilities to drive departmental success and initiatives. The Senior Director is responsible for driving seamless Care Coordination, Discharge Planning, Utilization Management, and Transitions of Care while partnering closely with physicians, nursing leadership, and executives to optimize patient outcomes, reduce readmissions, and ensure regulatory compliance.

    Qualifications:

    • Registered Nurse (RN) with acute hospital nursing experience required
    • Bachelor Degree in Nursing required
    • Director of Case Management in an Acute Hospital Setting required
    • Master’s Degree preferred
    • Certification in Case Management (CCM/ACM) preferred

    Responsibilities:

    • Oversees daily operations of the Care Management department, including Discharge Planning, Care Coordination and Social Services
    • Engages with post-acute care providers to improve transitions and reduce readmissions
    • Leads technology change management initiatives to enhance staff efficiency and patient outcomes
    • Collaborates with interdisciplinary leaders to achieve organizational goals and improve performance metrics
    • Develops and monitors operational budget to ensure fiscal accountability
    • Partners with Utilization Management for daily operations and Utilization Review Committee
    • Identifies opportunities for process improvement and implements action plans
    • Educates and mentors Care Management staff and interdisciplinary teams on best practices and outcomes
    • Ensures compliance with all federal, state, and regulatory requirements
