Position Overview:
Deliver high-quality, skilled nursing care to residents in a long-term care facility in Queens, NY. Collaborate with interdisciplinary teams to ensure patient well-being, adhere to care plans, and maintain compliance with state and federal healthcare regulations.

Qualifications:
- Education: Valid Registered Nurse (RN) degree.
- Licensure: Active New York State RN license.
- Experience: Minimum 1 year of long-term care experience.
- Technical Skills: Proficiency in patient care documentation and safe medication administration.
- Soft Skills: Strong communication, empathy, and adaptability in fast-paced environments.
- Additional Requirements: Immunity to Hepatitis B (proof required).
- Candidates must have proven experience and working knowledge in the following areas:
- EMR proficiency
- Conducting thorough clinical assessments
- Recognizing and responding to acute changes in condition and completion of SBAR
- Developing and updating baseline and comprehensive care plans
- Performing resident admission assessments and documentation
- Handling of controlled substances and documentation
- Coordination of care and supervising LPNs and CNAs
- Fall prevention, Elopement prevention, and response
- Conducting regular nursing safety rounds
- Nursing clinical documentation, task management, and follow-up
- Elder abuse prevention and reporting
- GG completion
- PRI and MDS
- Strong understanding of current best practices in long-term care settings
Key Responsibilities:
- Conduct patient assessments and implement individualized care plans.
- Administer medications and treatments per physician's orders.
- Monitor and document patient progress in EHR systems.
- Collaborate with healthcare teams to optimize resident outcomes.
- Ensure compliance with facility policies and regulatory standards.
