Compensation: $40.00-$58.67 (DOE).
Location: Central Oregon.
Schedule: Full-time (40 hrs/week) Monday-Friday, 8 AM-5 PM.
Travel: Local/regional travel by personal vehicle during the working day and extending throughout Central Oregon.

What You Get To Do: The Support Registered Nurse plans, coordinates, implements, and evaluates home health and hospice services, and is experienced in nursing, with an emphasis on community health education, providing services to meet the needs of individuals and families within their homes.
Complete a comprehensive and accurate assessment of the patient and family to determine needs.
Coordinate patient care with the patient’s case manager. Provide verbal or email updates as necessary.
Provide a complete physical assessment and history of previous illness (es) and use the health assessment data to determine the appropriate diagnosis and need for other disciplines and resources.
Provide nursing procedures, treatments, and end-of-life comfort measures as outlined in the care plan. Administer medications and treatments as prescribed by the physician.
Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and patients’ caregivers’ ability to learn. Adapt teaching styles to meet patients’ and families’ needs.
Communicates with the physician regarding the patient’s needs, reports, and changes in the patient’s condition; obtains/receives physician’s orders as required.
Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved in patient care.
Required Skills & Qualifications:
Valid Oregon RN License.
Ability to pass a background test, drug screen, and TB test.
Graduate of an accredited nursing school.
One to two years of recent medical care experience in an institutional setting is preferred. Ideal candidates will possess 1-2 years of home health or hospice experience.
Current automobile insurance is required. A valid Oregon driver’s license and reliable transportation.
Valid CPR Certification.
Strong communication and interpersonal skills.
Ability to work independently and collaboratively within an interdisciplinary team.
