Home Care Plus is growing, and we're looking for an experienced RN or LPN who understands the Lowcountry healthcare landscape and wants to make a meaningful impact outside of traditional bedside nursing.
If you've worked in home care, senior care, hospice, case management, care coordination, discharge planning, rehabilitation, or a similar healthcare setting, this could be the opportunity you've been waiting for.
This role combines clinical expertise, relationship-building, community outreach, and care coordination. You'll help local families navigate care needs while serving as a trusted resource for healthcare professionals throughout the Charleston area.
Why This Role Is Different
This is not a hospital floor position.
You won't be tied to a bedside, working nights, weekends, or holidays. Instead, you'll have the flexibility and autonomy to manage your schedule while building relationships throughout the community and helping families access quality care.
What You'll Do
Conduct in-home assessments and develop personalized care plans
Meet with clients and families to understand their needs and recommend solutions
Build and maintain relationships with hospitals, physician offices, rehabilitation centers, senior living communities, hospice organizations, and other referral partners
Represent Home Care Plus at community events, networking functions, and healthcare meetings
Support caregiver recruitment, onboarding, and quality assurance visits
Serve as a clinical resource for clients, families, caregivers, and referral partners
We're Looking For Someone Who:
Holds an active South Carolina RN or LPN license
Has experience in home care, hospice, case management, discharge planning, senior care, rehabilitation, or a related healthcare field
Knows the Charleston, Mount Pleasant, and surrounding Lowcountry healthcare community
Is comfortable building professional relationships and networking within the community
Communicates confidently with physicians, social workers, case managers, and families
Is self-motivated, organized, and able to work independently
Wants to grow with a locally owned company that values initiative and leadership
Preferred Qualifications
5+ years of nursing experience
Geriatric experience required
Experience with business development, community outreach, admissions, or care coordination
Home care industry experience strongly preferred
