    Glory Hospice has over 40 years of clinician experience in pain and symptom management. We are currently looking for a social worker/MSW to join our growing team and provide excellent care in San Diego with an emphasis on North County.

    ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

    Responsibilities:

    Collaborate with the hospice interdisciplinary team, which includes physicians, nurses, and other healthcare professionals, to develop hospice care plans

    Conduct assessments of hospice patients' social and emotional needs and develop appropriate interventions

    Provide individual and group counseling to hospice patients and families on issues related to hospice care, chronic illness, and end-of-life care

    Connect hospice patients and their families with community resources and services

    Case-manage hospice patients who require ongoing support and assistance

    Document all interactions in the hospice patient's medical record according to departmental standards

    Participate in hospice team meetings and quality improvement initiatives as needed

    Qualifications:

    Master's degree in Social Work from an accredited institution

    2+ years of experience working in hospice, home health, or with chronically ill patients preferred

    State licensure as a Clinical Social Worker or eligibility to obtain licensure required

    Knowledge of hospice care, medical terminology, and medical social work theory and practice

    Strong communication and interpersonal skills

    Ability to work collaboratively as part of a hospice interdisciplinary team
