JOB Description
Establish and maintain good relationships with hospitals, nursing services, social services, therapy offices and various other medical and non-medical support services
Communicate personally with patient care managers to ensure standards of care are being met and that patient needs are being identified and met on an ongoing basis
Chat with hospitals, doctors, prospective clients and their families about the home care services the agency provides
Gather and prepare information in advance of patient transfers; track data related to such transfers and compile information into reports such as statistical reports
Keep communication smooth between hospitals and doctors and home care agencies by continually reporting patient progress and therapies received
