  • Responsibilities

    To administer quality, skilled nursing care to clients on an intermittent basis in the home.  Perform this care in accordance with the physician’s orders and the Plan of Care under the director and supervision of the Clinical Services Director.

    ESSENTIAL JOB FUNCTIONS:

    • Approach all encounters with clients and associates in a friendly, service oriented manner
    • Maintain high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working as appropriated.
    • Comply at all times with regulations and standards to encourage safe and efficient property operations.
    • Assess home care patients to identify the physical, psychosocial and environmental needs of clients.
    • Demonstrate knowledge and proficiency in communicating, identifying, reporting and documenting problems, adverse reactions or possible complications related to client care.
    • Demonstrates knowledge of emergency procedures and responds appropriately as well as reports life threatening situations with appropriate response.
    • Follows all environmental and safety guidelines/standards per RHC policy and procedure.
    • Identify and recommend other disciplines needed for quality care of the client.
    • Document evaluation of nursing interventions including client response.
    • Assess client/caregiver’s knowledge base of disease process, treatment regimen and/or medication administration and assist with care plans.
    • Provide client skilled services under the direction of licensed physician using current nursing standards.
    • Monitor assigned cases to ensure compliance with requirement of third party payers.
    • Attend all required in-service programs and maintain continuing education units for licensure renewal as applicable to State regulations.
    • Assist with Medication Management.
    • Participates in the RHC Improvement process.
    • Perform other duties and tasks as assigned or required.

    Required Skills

    • Must be able to read and write in English and follow written and verbal instructions in English effectively.
    • Proficient in computer use (Microsoft Outlook, Word, Excel, etc.)

    Required Experience

    Licensed Practical Nurse and two years’ experience.  Experience in working with seniors is important.  

    SPECIAL REQUIREMENTS/CERTIFICATIONS:

    Licensed as LPN, CPR and First Aid Certified, and Universal Precautions trained.  Experienced working with Microsoft Office and other software programs.  Reliable transportation required with proof of valid automobile insurance and driver’s license and visual acuity and sufficient dexterity to operate a motor vehicle.
