    The Palliative Care LCSW will provide essential emotional support, counseling, and care coordination, conducting home visits to help clients and their families establish effective palliative care at home. This position will support the Central/South Fairfield County Region.

    Responsibilities:

    Performs initial psychosocial assessments and assists in the development and implementation of goal-directed palliative care plan and next steps.

    Conducts ongoing reassessments, goals of care conversations of patient/family needs and counseling as required.

    Provides short term crisis intervention and individual or family counseling when indicated.

    Provides services, under the direction of a physician (who approves plan of care).

    Referrals to community resources as appropriate.

    Intervening in client crisis situations.

    Educating clients and families about their treatment plan and the resources and support systems available to them.

    Addressing a client’s physical, mental, social and spiritual well-being in all disease stages.

    Conducting home visits to help clients and their families establish effective palliative care at home.

    Providing psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy.

    Observes, assesses information regarding psychosocial, emotional, physical and financial conditions affecting the patient and family. Assist in understanding the significant social and emotional factors related to client’s health problems.

    Assumes the active role of advocate for the patient/family/caregiving unit.

    Conduct home visits to help clients and their families establish effective palliative care at home.

    Provide counseling and psychotherapy for individuals, couples, and families.

    Provide psychosocial education to patients and family caregivers about coping skills, hospice and palliative care philosophy, and nonpharmacological symptom management strategies.

    Address psychological and social needs to support patients living with serious illness, and their families.

    Provide grief counseling, family counseling, and assist families by coordinating care details and research helpful resources and benefits.

    Provide education to community regarding Palliative care, services, philosophy.

    To offset census at times, may be requested to see home health patients

    Experience and Qualifications

    Master’s degree in social work from a school accredited by the Council on Social Work Education

    LCSW in the state of Connecticut required

    At least two years of social work experience, after a master’s degree has been obtained

    Palliative Care or Hospice experience required
