    The Chronic Care RN provides in-home nursing assessments to a population of chronic and stable patients, in accordance with physician’s orders, as part of the Connecticut Dept of Social Services’ Medicaid Waiver program. This is a Per Diem position for the Southern Fairfield County region.

    Responsibilities:

    Provide health maintenance home visits to a population with chronic conditions.

    Under MD orders, competently conduct full head-to-toe assessment, medication reconciliation, and other assessments as needed.

    Identify and document patient and family needs in electronic medical record utilizing a tablet. Report findings to M.D., as appropriate.

    Perform evaluation of patients’ progress, promptly acts on any changes made to patient’s plan of treatment or condition

    Appropriately utilizes and maintains all nursing equipment and supplies needed for patient care.

    Counsels patients and families on the basic principles of wellness including information on related fields such as nutrition, safety, education, mental health, dental care, and overall health and wellness aspects.

    Assesses patients at risk for existing or potential situations of neglect and initiates referral to appropriate community agencies.

    Knowledgeable and compliant with RVNAhealth policies and procedures, HIPAA, state regulations, Corporate Compliance, CHAP accreditation standards.

    Provide exceptional customer service to both clients and caregiver staff by responding to issues and/or requests in a timely, professional manner

    Qualifications:

    Current RN license in the State of Connecticut

    Home Care and/or Hospice experience (1-year).

    Able to communicate clearly with patients, families, visitors, leadership and others.

    Able to teach patients and families in accordance with the nursing plan of care.

    Interest in working with low-income populations, particularly seniors.

    Willingness to work as a team member.

    Competent computer skills.

    Must have reliable transportation.

    Current CPR certification.

    Must be able to travel to, enter and function in the homes/facility of any patient to assess and provide care to all patients.
