    The Physical Therapist is a qualified professional who directs, supervises, evaluates and provides physical therapy services to patients accepted for care by the RVNAhealth as prescribed by the attending physician. The therapist is responsible to the Director of Rehabilitative Services for fulfilling the needs of the agency in the area of his/her expertise. This position will cover our North region from Bethel to Kent

    Responsibilities: • Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan in conjunction with RN (if applicable), patient, family, and physician. • Perform Oasis assessment visits accurately and at appropriate time points. • Instruction of patient, family and other agency health care personnel in the patient's treatment regime when indicated • Supervision of Home Health Aides as needed by the agency and when such personnel are participating in the patient's therapy regime. • Provide proper Supervision and oversite of PTA’s and ensuring proper communication and documentation. • Assists patients to reach a level of function to enable them to remain safely in their home environment while reaching national standard for outcomes and maintain fiscal responsibility. • Obtains written physician's order for Physical Therapy every sixty (60) days. • Identify and make appropriate referrals to next level of care as needed • Reports in writing the patient's condition to the physician at least every sixty (60) days. • Serves as a consultant to other health team personnel regarding physical therapy for individual patients. • Maintains case load and productivity. • Maintains Continuing Educations courses as mandated by the State of Connecticut. • Knowledgeable, completes and compliant with all agency and regulatory procedures including but not limited to Corporate Compliance, HIPAA and Medicare conditions of participation. • Ensures payment for care by meeting payor requirements and/or obtaining authorizations. • Completes quality documentation within required agency and/or regulatory timeframes. • Maintains Standard Precautions Infection control and proper bag technique. • Maintains clinical Competency in all areas of scope of practice

    Qualifications: • Graduate with at least a baccalaureate degree of an accredited school of Physical Therapy approved by the American Medical Association Council of Medical Education with collaboration with the American Physical Therapy Association. • Current license to practice as a Physical Therapist in Connecticut. • At least two (2) years' clinical experience. • Knowledge of physical therapy methods with particular skill in casework in the home setting. • Ability to adapt this knowledge and skill to a home setting. • Knowledge and understanding of the principles of home care and community health administration and practice in the home setting. • Ability to establish good working relationships with other professions. • Ability to collaborate effectively with community agencies. • Ability to teach and communicate goals to the home care team - in the home setting.
