    The OASIS Plan of Care RN is responsible for OASIS initiatives with primary responsibilities that include acting as a liaison between outsource agency, finalization of OASIS and Coding as needed, assuming responsibility for clinician OASIS assessment, action plan development, training and evaluation to assure on-going improvement in all outcome measures as collected in the OASIS Assessment tool and management of the workflow process through HCHB. This position is an educator and resource to the clinical home health team in regard to OASIS.

    Qualifications/Expectations

    Graduation from an accredited School of Nursing.

    Currently licensed to practice as an RN in the State of Connecticut.

    Has a minimum of one year of nursing experience.

    Ability and willingness to follow Agency policies and procedures and to work as a team member supporting the agency mission, vision, and values.
