The LCSW/LMSW is a qualified professional, who delivers varied social work services to Hospice and Palliative patients and their families. The LCSW/LMSW is an integral part of the Hospice and Palliative IDT, who provides initial and on-going emotional, spiritual, psychosocial, grief and bereavement assessment and support, funeral planning, community education, outreach and referral support and service. This position will cover the Southern Fairfield County Region (Stamford, Greenwich, Darien).
Responsibilities:
• See Clients initially within the required regulatory timeframe and within the RVNAhealth Policy timeframe. • Performs initial psychosocial, emotional, spiritual and bereavement assessments and assists in the development and implementation of goal-directed IDT care plan and next steps. Performs Suicidal risk assessments and care planning as needed. • Conducts ongoing reassessments of patient/family needs and counseling as required. • Provides short term crisis intervention and individual or family counseling when indicated. • Provides services, under the direction of a physician (who approves plan of care). Referral to Community resources as appropriate. • Participates as a member of the Bereavement Team as assigned. • Observes, assesses and brings to IDT conferences information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family. Assist other IDT in understanding the significant social and emotional factors related to client's health problems. • Assumes the active role of advocate for the client/family and caregiving unit. • Develops and maintains contact with appropriate community agencies and services to promote interagency cooperation and to facilitate related referrals. • Help the client and his/her family to understand, accept, and follow the medical recommendations and provides services planned to restore the client to optimum social and health adjustment within his/her capacity. • Quality documentation for all client visits, contacts, referral actions, and assessments, including comprehensive psychosocial, emotional, and spiritual assessment clearly and concisely in a timely manner in the agency electronic medical record. RVNAhealth expects Point of Care (POC) documentation, and that documentation is completed within the required agency timeframe. • Provides ongoing counseling related to issues of death and dying to the client, family and caregivers as needed.
• Attends staff meetings, IDT, case conferences, educational programs, and other meetings as assigned and appropriate and assigned.
• Participates in orientation and other education programs as assigned, including meetings and programs in the Community. • Adheres to all Hospice/Palliative Care policies. • Assumes responsibility for own personal and professional development and maintenance of skills in social work. • Exhibits Hospice/Palliative philosophy in all job-related roles. • Other duties as assigned by the Support Services Manager. • Follow and utilize appropriate infection control and proper bag technique. • Knowledgeable and compliant with all RVNAhealth, financial, and regulatory procedures including but not limited to Corporate Compliance, HIPAA, Medicare and Medicaid Home Health and Certified Hospice COP’s, and payment for private pay services not covered under Hospice Benefit. • Establish respectful, positive working relationships with other employees and volunteers in the delivery of client services. • Participate in on call rotation.
Experience and Qualifications
Master’s degree in social work from a school accredited by the Council on Social Work Education
One year of experience in Hospice/Hospital/Home Care setting preferred.
At least two years of social work experience, after a master’s degree has been obtained
Experience and ability in end-of-life client and family assessment and care planning including but not limited to Advance Directive/Living Will/POA Care Planning, End of Life physical, emotional, and spiritual symptom control, and Client and caregiver advocacy and support skills.
Excellent communication skills and ability to work successfully with others during times of increased stress.
Acceptance of and adaptability to different social, racial, cultural and religious modes.
Is computer literate and able to document at point of care.
Is able to drive and travel in all types of weather and environmental conditions.
