Independent Living Coordinator – Juniata County

Pay Rate based on education and experience $18.50 - $19.50 per hour

ABOUT YOUR WORK DAY:

Schedule is typically 8am to 4:30pm, M-F (is dependent on the families’ needs)

You will be working with children, youth, and families from Juniata County in conjunction with Children & Youth Services. Here are some items you will be responsible for:

1.) Initiate and maintain contact with youth who are referred through CYS

2.) Have youth complete assessment and assist in developing Independent Living Goal plans within one month after first contact

3.) Review Independent Living plans with each youth quarterly

4.) Prepare and complete required documentation; including case notes, in compliance with regulations, company standards, and the policy and procedure manual.

5.) Be available and prepared to provide court testimony in a professional manner as scheduled.

6.) Schedule, plan, and complete group activities based on participating youth need and availability

DO YOU HAVE THE FOLLOWING REQUIREMENTS?

· A valid Pennsylvania Driver’s license with good driving record

· Bachelor’s Degree in related field; or

· Bachelor’s Degree in unrelated field with minimum 12 credits in a Human Service related course; i.e., psychology, sociology, counseling, etc.
