About the Role:
MedPOINT Management in Sherman Oaks, CA is looking for a dedicated Re-Admissions Case Manager to join our growing healthcare team. In this impactful role, you'll play a critical part in coordinating patient care transitions and reducing preventable hospital readmissions. Responsibilities:
Monitor and manage high-risk patients post-discharge to prevent unnecessary hospital readmissions
Coordinate care transitions between hospitals, skilled nursing facilities, and outpatient providers
Conduct telephonic and in-person patient assessments to identify barriers to care and social determinants of health
Collaborate with interdisciplinary care teams including physicians, social workers, and utilization management staff
Develop and implement individualized care plans focused on reducing readmission risk
Track and document case management activities in electronic health record (EHR) systems
Educate patients and caregivers on discharge instructions, medication adherence, and follow-up care
Requirements:
Active RN or Licensed Clinical Social Worker (LCSW) license in the state of California
2+ years of case management, utilization management, or discharge planning experience
Strong knowledge of managed care, IPA, or health plan operations preferred
Experience with care transitions, readmission reduction programs, or chronic disease management
Proficiency with EHR/case management software and Microsoft Office Suite
Excellent communication, critical thinking, and organizational skills
CCM (Certified Case Manager) certification is a plus
