    The Registered Nurse Case Manager - (RN) is client focused, has a passion for client outcomes, with the ability to motivate and influence those within their area. The RN Case Manager ensures that the best possible care and services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures. Field responsibilities may include supervising medical staff, ensuring that the health and safety of all clients, communicating with clients family members, working with OASIS, utilizing ICD 9/10 coding procedures.

    -Ensures that LPNs, RNs and Aides are properly oriented and trained to meet the clients' needs in accordance with agency policies and procedures

    -Instructs clients, family members and primary care givers as needed to ensure the maintenance of or acquisition of optimal functioning level for each client

    -Develops and implements a Plan of Care based on nursing diagnosis, client and physician input that includes medical interventions and measurable goals or outcomes

    -Completes and submits OASIS assessments, reassessments and transfers, resumptions of care, discharges and significant change in condition in accordance with agency, state and federal guidelines

    -Completes nursing assessments of all in home clients within specified deadlines

    -Provides new or continued services by educating and creating relationships with members of the health care field

    Requirements

    -Current Registered Nurse (RN) license in the state of application

    -Minimum of one year (RN) Home Health Care Experience or LTC Experience a plus

    -Oasis knowledge/experience a plus

    -ICD9/10 knowledge a plus
