The Heart of This Role
The Referral Coordinator ensures patients receive the specialty, diagnostic, and support services they are referred to without confusion, delay, or missing information.
This role is essential to continuity of care. Referral Coordinators track referrals from start to finish, communicate with patients and outside providers, and make sure critical information makes its way back to the care team.
This is not just paperwork.
It is coordination, communication, and follow-through that directly impacts patient outcomes and trust.
What This Role Looks Like, Day to Day
In this role, CONNECTION means:
You communicate clearly with patients about referral appointments, locations, and expectations.
You reduce anxiety by explaining what happens next and who to contact with questions.
You collaborate closely with care teams to remove barriers to care.
You treat each referral as a real person waiting for services, not a task to check off.
OWNERSHIP means:
You track referrals through completion and follow-up.
You ensure referral documentation is accurate, timely, and complete in the EHR.
You close referral loops by obtaining consult notes or confirmation from outside providers.
You identify delays or incomplete referrals and escalate appropriately.
You support quality and compliance standards through consistent documentation and follow-through.
