Intake Coordinator/Social Worker – Independence Home Health LLC
Independence Home Health LLC is seeking a dependable and detail-oriented Intake Coordinator /Social Worker to join our team. The ideal candidate is organized, adaptable, and committed to providing excellent service to patients and healthcare partners.
Responsibilities
Manage and coordinate patient intake and referrals
Communicate with patients, families, physicians, and staff to obtain necessary information
Review and verify documentation for accuracy and completeness
Maintain organized records and follow compliance guidelines
Qualifications
High school graduate and at least 18 years of age
Minimum two (2) years of work experience, preferably in a healthcare setting
Detail-oriented with strong communication and customer service skills
Knowledge of medical terminology, insurance processing, and prior authorizations preferred
Familiarity with home care regulations and community resources
Proficient in Microsoft Word and Excel
Strong organizational, problem-solving, and multitasking skills
Home Health Aide (HHA) or Certified Nursing Assistant (CNA), or willingness to obtain certification
Valid driver’s license and insured vehicle in accordance with agency requirements
