We are looking for an experienced Admissions/Intake Coordinator with a desire to make a difference in someone's life to join our team. A Licensed Vocational Nurse is preferred, but not required.

We are looking for a Hospice Intake/Admissions Coordinator with a desire to make a difference in someone's life to join our team.

The Hospice Intake/Admissions Coordinator's primary responsibility is to meet the needs of our patients and our referral sources by coordinating the admissions process with staff, referral sources, patients, and services such as transportation and DME. QUALIFICATIONS INCLUDE:

• One year of hospice or healthcare experience
• An understanding of the hospice philosophy
• Strong interpersonal, customer service, and telephone skills
• Knowledge of Medicare Managed Care, Medi-Cal coverage, and other private insurance
• Medical Terminology, ICD-10, multi-tasking, problem solving, and basic computer skills
• High school diploma or equivalent
• Current Driver's License and automobile insurance
