  • Responsibilities

    Regional Family Care Coordinator provides guidance and case consultation to the team. The Care Coordinator provides support to case managers on complex cases and facilitates psychoeducational classes. It requires a knowledge of local resources and an ability to build relationships and broker services with local providers.

    We are looking for a Regional Family Care Coordinator who must have:

    • Master’s Degree in Social Work, Mental Health, Counseling, Psychology, or related human services discipline.
    • Two (2) years’ experience working with at-risk youth and their families which may include case management or classroom instruction.
    • Must possess a valid Florida Driver’s License and Auto Insurance

    Job duties and responsibilities include:

    • Conduct individual and/or group psychoeducational classes with youth and families in assigned program(s) at youth’s home, BAYS office or other appropriate alternative location.
    • Attend regularly scheduled case reviews/service team meetings within assigned circuit(s) to provide assessment and input during initial and ongoing case planning activities.
    • Maintain relationships and communication with community provider agencies and continuously update community resource/referral lists for assigned circuits.
    • Maintain documentation related to consultation activities within appropriate time frames, Agency, and Quality Improvement standards.
    • Manage daily responsibilities by working independently without direct supervision.
    • Perform all other related duties as assigned.

    Job Essentials:

    • Provide case oversight, leadership, and support to assigned circuits.
    • Review all incoming cases independently to identify cases that may require additional support or wrap around, to proactively prepare necessary resource information.
    • Coordinate and determine with Case Managers, Senior Case Managers and Circuit Supervisor which cases require additional support or oversight due to multi-agency involvement.
    • Offer and provide ongoing case review to support Case Managers.
    • Make appropriate referrals to community agencies and follow-up within timeframe defined by Quality Improvement Standards.
    • Drive daily for client visits, school checks and meetings
