JM2 Health is looking to hire a Registered Dietitian to provide comprehensive diabetes self-management education onsite within Ascension Wisconsin.

    Job Details:

    Locations: Ascension Wisconsin- Columbia St Mary Milwaukee/Columbia St Mary Ozaukee/Brown Deer. Positions also available at Ascension Columbia Saint Mary, Saint Francis, Saint Joseph & Mayfair Rd Endocrinology. Occasional travel to other locations within SE Wisconsin may be required based on staffing needs.

    Hours- Full Time, salary;

    Job Summary:

    The Diabetes Educator/Certified Diabetes Care and Education Specialist (CDCES) delivers comprehensive services that integrate clinical and self-management aspects of diabetes. They are an integral part of the interprofessional team and provide collaborative, comprehensive and person-centered care and education conducive to behavior change and improved quality of life across the lifespan. The Diabetes Educator/CDCES supports and advocates for people affected by diabetes to optimize quality care. The Diabetes Educator/CDCES promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes.

    Role and Responsibilities:

    Integrate skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management of diabetes into clinical practice.

    Provide quality diabetes self-management education and medical nutrition therapy in individual and group settings based on assessed needs. Utilize appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.

    Complete comprehensive assessments for each patient including emotional and behavioral health, interpret personal health data, develop individualized care plans based on the patient’s assessed needs and goals, and promotes successful self-management

    Apply current research and evidence-based care to practice and engage in lifelong learning to serve as a role model of professionalism.

    Actively participate in the quality improvement (QI) process and adapt practice/processes based on QI finding.

    Document all visits in the Electronic Health Record and outcomes database according to guidelines promptly.

    Advocate for and support technology-enabled diabetes education and care.

    Collaborate with other members of the diabetes care team in developing person-centered diabetes plans.

    Contribute to the achievement of established departmental goals and objectives and adhere to department policies, procedures, quality standards and safety standards.

    Participate in meetings and represent the healthcare organization in community outreach efforts as appropriate.

    Perform other duties as assigned.

    Minimum Qualifications

    Bachelor’s degree in dietetics required

    Registered with the Commission on Dietetic Registration (RDN) required

    Certified Diabetes Care and Education Specialist (CDCES) or BC-ADM credential preferred (If not already credentialed, will be required to sit for the exam within 2 years of hire.)

    Diabetes self-management education and support experience preferred

    Knowledge, Skills and Abilities

    Strong problem solving and critical thinking skills

    Flexibility to work days, early evenings, and some weekends as needed

    Excellent interpersonal and group communication skills

    Ability to effectively communicate with patients, public, medical staff and physicians

    Ability to work with a diverse patient population and have exceptional customer service skills

    Physical Requirements and Working Conditions

    Must be willing to travel to various locations within an approximate radius of 25 miles

    Must be able to sit, stand, walk, lift, carry, push/pull, bend, squat, twist, rotate, kneel and reach occasionally throughout the workday

    Operate equipment necessary to perform the job
