Abacus Corporation is seeking a Diabetes Prevention Nurse, This position supports diabetes prevention and health education initiatives designed to help individuals understand their risk for diabetes and make sustainable lifestyle changes.
The ideal candidate is a compassionate, organized healthcare professional who enjoys patient education, community health, and helping individuals take an active role in improving their health.
What You'll Do
- Support diabetes prevention and wellness initiatives within the community.
- Provide health education and guidance related to diabetes prevention, healthy lifestyle choices, nutrition, and physical activity.
- Work with individuals to encourage positive and sustainable behavior changes.
- Assist with participant outreach, engagement, and follow-up.
- Help monitor participant progress and maintain accurate program documentation.
- Provide education and resources to individuals regarding diabetes risk reduction.
- Collaborate with healthcare professionals, community organizations, and program staff.
- Maintain confidentiality and follow applicable healthcare and program requirements.
- Participate in meetings, training, and other program activities as needed.
- Complete other duties related to diabetes prevention and community health initiatives.
What We're Looking For
- Registered Nurse (RN) with an active nursing license.
- Experience in public health, community health, health education, diabetes prevention, or a related healthcare setting preferred.
- Strong communication and interpersonal skills.
- Ability to educate and engage individuals from diverse backgrounds.
- Strong organizational and documentation skills.
- Ability to work independently while also collaborating effectively with a team.
- Comfortable working with community members and healthcare partners.
