Job description:
Responsibilities
Initial in-home assessment, development of the plan of care as applicable and regularly supervises the needs of the client every 90 days.
Manage in-home aides to ensure the level of care is being rendered according to the plan of care.
Observe and report changes in the patient’s physical and/or emotional condition.
Initiate preventative procedures as appropriate for the patient’s care and safety and effecting changes in environment to ensure patient safety and security.
Maintain confidentiality of patient and agency matters.
Submit required documentation in a timely manner.
Assist the administrator in decision making and policy formation as it pertains to the home care services.
In addition to scheduled hours, participating in weekend/holiday call schedule, as needed.
Qualifications
Graduate of an accredited school of nursing
Valid and current RN license in the state of North Carolina
CPR & 1st aid certified, good standings on nurse registry
1-3 years of nursing experience (home care experience, preferred)
Supervisory skills, a plus
Excellent communication skills, flexible and able to interact positively with the rest of the staff
Strong sense of initiative and urgency in addressing unexpected patient crises
